Service Officer - Home From Hospital
Job Summary
Hours of work: 35 hours per week
Annual Leave: 20 days, plus all statutory bank holidays and one compulsory day off from Age UK Bury. In addition, we are offering 3 days paid leave for you to undertake voluntary work for a charity of your choice.
Contract: Temporary subject to funding. The service is currently subject to an ongoing rolling renewable contract. Funding has been secured for 2024/25.
Salary: £21,699.30
Job Location: Fairfield General Hospital, Rochdale Old Road, Bury, BL9 7TD although maybe required to work at other locations within the borough.
Car Usage: There is a requirement for the Service Officer to have a full, clean driving license and access to a vehicle.

- We are looking for a Service Officer who is supportive, creative and strives for excellence.
- The Home from Hospital Service is a new service with the aim of supporting hospital discharges and reducing readmissions back to hospital.
About You
You will be comfortable working with and communicating with people aged 50+. You will be highly organised and motivated to help individuals transition from being in a hospital environment to returning and managing at home.
