Our friendly and supportive Hospital Discharge Support Team provides transport from Stoke Mandeville, Amersham, Wycombe and Wexham Park after a hospital stay and also offers support to settle recent patients back into their homes. We can also offer short term, practical support in order to prevent readmission to hospital.
How to access our Hospital Discharge Support Service
This service supports people in two ways:
Our friendly discharge Support Team will transport people home from Stoke Mandeville, Amersham, Wycombe and Wexham Park Hospitals and help them settle back home after their hospital stay. We have two wheelchair accessible vehicles working across the county to support safe discharge home. We can also support with a light shop, small domestic tasks and ensuring people are settled back at home with something to eat and drink.
Our Community Support Team can also provide recent patients with short term support at home to help with shopping, light housework and emotional support in order to maintain their independence at home. Our Community Support Team can support at home to help with:
• Shopping – You will need cash and a shopping list to enable us to do this.
• Prescription and Medication collection
• Light housework such as hoovering and changing bedding – Please note we are unable to clean bathrooms.
• Laundry
• Assistance with paperwork organisation
• Advice and Signposting
Referrals to our Discharge Team are via NHS staff, our community team can accept referrals from individuals, carers, voluntary sector organisations, health care professionals and social care colleagues.
