Post Discharge Settling Service

The Bromley Well Post Discharfge Settling Service is a free service that supports vulnerable or mildly frail older people and vulnerable adults who live in the community, live alone, or do not have anyone to provide assistance.

The service supports people for the first 24 hours after discharge from hospital and focuses on providing companionship and recuperation assistance to enable people to regain their confidence and independence in their home environment and prevent readmission to hospital.

Access to the free service is only available via:

• Hospital Staff from Wards, the Clinical Decision Unit, the Discharge Lounge, the Emergency Department or Urgent Care at Princess Royal University Hospital
• Hospital Staff at the Frailty Unit at Orpington Hospital
• the Transfer of Care Bureau
• Bromley Local Authority Staff, including GPs and Social Care Practitioners
