Role Description

You will support older people to make a safe discharge from hospital and provide them with the practical and emotional support they need to recover at home.

Specific Responsibilities

The role will involve:

  • Visiting clients at home, providing practical and emotional assistance, in line with the support plan, for a period of up to six weeks.
  • Carrying out tasks that may include: making sure their home is safe and warm for them on arrival home from hospital; collecting prescriptions; preparation of light meals and snacks; light housework duties; washing up; helping to sort out paperwork that may have accumulated during their stay in hospital; shopping for essential items and giving encouragement to practice OT/physiotherapy exercises.
  • Attending follow-up medical appointments with the patient; booking transport if required. 
  • Reporting back any concerns to the Service Co-ordinator, highlighting any additional support needs that may have arisen.

Please note this role does NOT include any personal care.

What We're Looking For

  • Skills and willingness to work face to face with clients, in their home setting
  • Ability to confidently volunteer independently, using their own initiative
  • Ability to show empathy and compassion to clients
  • Ideally suited to people who have a background, or a genuine interest in Health and Social Care. This is not essential however, as all training will be provided.
  • Willingness to attend training and other events.
  • Willingness to undergo a DBS check.

Time Required

Minimum of two hours per week, for a period of up to six weeks.
