We understand that living with and managing a long term health condition independently as you get older can sometimes be a struggle and confusing with lots of different health professionals involved. Our Integrated Care Service offers coordinated support and interventions to help avoid admission to local health and care services and help you remain at home independently for as long as possible. We can talk to professionals on your behalf and recommend services and support including practical support that can make a difference. Our trained staff listen to your needs and help you to remain resilient, supporting you to take control of your own health.

What we offer

Eligibility

- Over 50 years old (all adult age for hospital discharge)
- Live in Blackburn with Darwen or registered with a Blackburn with Darwen GP
- Struggling to manage long-term conditions
