Community Connector
We're looking for passionate Community Connectors to join our dedicated team at the Royal United Hospitals NHS Foundation Trust. In this role, you will directly impact the lives of older adults, helping them transition from hospital to home by identifying their wellbeing needs and connecting them with essential services.
What You’ll Do:
- Engage with patients, their families, and healthcare professionals to tailor support services effectively.
- Manage the distribution of HELP packs and oversee a team of volunteer Connectors, ensuring those we serve are well-supported during their transition.
